Set your baseline. Our assessment leverages a combination of self-report data, previous medical records, biometric and biomarker analysis, functional fitness testing, cognitive evaluation and more to learn about your unique physiology, risk profile, and current health behaviors.
VITALITY Report
The Vitality Report contextualizes the many data points derived during your assessment. Think of the Vitality Report as your current health and performance baseline in a comprehensive report card. At the end of the report, your Performance Physician provides a interpretation or key finding and highlights potential areas for focus (including advanced diagnostics and therapies).
Performance Plan
Whereas the assessment provides awareness and education, ongoing members of the practice focus on execution. Your Performance Physician learns about your values, goals, current health beliefs, daily routines, sources and strategies of motivation, physical and psychological limitations, support system, and available resources—all critical variables for health behavior change. You and your Performance Physician then co-design a long-term strategy and Performance Plan that align with your unique vision of optimal health, wellbeing, and performance. The Performance Plan consist of prioritized performance goals, prioritized health goals, habit development strategies for sustained behavior change, a custom fitness plan, suggested therapies and referrals, a schedule for interval testing and monitoring, a process road map with future considerations, and tools and resources to enable continued education and action.

Training Plan
Performance Medicine is predicated on the principle that movement is medicine—health promoting and healing for both body and mind. A foundational component of your prescription for better health and performance is a custom, multimodal training plan. And proper training to stimulate the proper physiologic adaptation requires a proper prescription.
